– El antropólogo italiano, colaborador de la Clínica Hospital del Pueblo “Anna Seethaler” en la ciudad de Oaxaca, plantea la necesidad de que la política pública tenga una visión más abierta para combatir efectivamente las enfermedades.
Las enfermedades no son sólo biológicas, hay componentes sociales, culturales y económicos que las propician y permiten que continúen presentándose, principalmente en sectores de la población vulnerables. Es decir, la desigualdad y la marginalidad social también son unas de las causas de algunos padecimientos.
Fabio Pettirino, colaborador en la ciudad de Oaxaca de la Clínica Hospital del Pueblo “Anna Seethaler” y antropólogo social, sostiene que “cada enfermedad tiene una denuncia social incorporada”.
El especialista explica que los antropólogos siempre se han preocupado de hechos sociales y llegaron también a estudiar las medicinas tradicionales y después de mucho tiempo también la medicina alopática que es la medicina occidental, la que ahora es la “medicina hegemónica”; ésta última, dice que tiene un enfoque reduccionista o reduce todo a lo biológico y sólo medicaliza a las enfermedades.
Por ejemplo, menciona la tuberculosis. Es cierto que es un hecho biológico en cuanto al hecho de que es causada por un germen; pero esta enfermedad se da principalmente cuando hay condiciones de privación que hacen bajar las defensas inmunitarias, y aunque muchas personas pueden curarse con antibióticos, las condiciones sociales en la que vive provocarán que vuelva a enfermarse.
“Entonces sí es cierto, que es un hecho biológico, pero al mismo tiempo está producido por condiciones de desigualdad social. Otro ejemplo es la diabetes, la cual es una condición crónica y es una condición de este desequilibrio fisiológico, pero al mismo tiempo, hay causas y consecuencias sociales que la producen”.
Fabio Pettirino, antropólogo originario de Turín, Italia, detalla que cada enfermedad al ser analizada desvela los mecanismos sociales que la producen y que incluso, provocan que algunos pacientes no puedan apegarse o seguir el tratamiento que un médico les ha recetado, hay sea porque no entienden las instrucciones o por cuestiones económicas: que no tengan el dinero para comprar el medicamente, o incluso, que no cuentan con un refrigerador para conservarlo, como en el caso de la insulina que se utiliza en la diabetes.
“Un ejemplo son los riñones de las personas que viven alrededor del Lago de Chapala. Esas personas tienen un tamaño reducido de los riñones por la contaminación del agua que están obligados a tomar de este lago, y no es porque beban esa agua, es porque la usan para lavar la ropa, para regar sus hortalizas, y de alguna forma se incorpora esta desigualdad social en los niños con riñones de un tamaño reducido. Esto significa que ya la desigualdad social se ha vuelto en un hecho biológico y ese riñón, chiquito, pues es un hecho biológico, pero al mismo tiempo es una denuncia social incorporada porque ya se ha vuelto en una biología local”.
Fabio Pettirino opina que la política pública del gobierno de Oaxaca y el gobierno federal debe tener una visión más abierta sobre las enfermedades, e investigar las verdaderas causas que las están causando más allá de los hechos biológicos.
De aquí surge, dice, el concepto de medicina comunitaria que se enfoca no tanto en el tratamiento de la enfermedad, sino que en las circunstancias y contexto social en el que se da esa enfermedad. “Cada enfermedad”, sentencia, “tiene una denuncia social incorporada”.
